An initial inflammatory signal triggers a localised sensation of itch. The signal travels up unmyelinated C-nerve fibres directly to the sensory cortex, forcing an automatic, behavioural scratch response.

The danger of the scratch response

Scratching offers brief relief by substituting pain for itch. But the physical friction damages keratinocytes, which release a flood of pro-inflammatory cytokines and histamines. That further damages the skin barrier, creating an even more intense itch signal.
